HIPAA Policy

THIS NOTICE DESCRIBES HOW YOUR PERSONAL INFORMATION COLLECTED VIA ONLINE AND MOBILE WEBSITES, PLATFORMS, SERVICES, AND APPLICATIONS THAT WE OWN, OPERATE, OR UTILIZE MAY BE USED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

  1. Health Information

“Health Information” is information that identifies you as a current or former Central Iowa Family Institute “CIFI” client/patient/guardian that relates to your/your dependent’s health or condition, your health care services, payment, or coverage for those services.

The privacy of Health Information is governed by the applicable Notice of Policies and Practices to Protect the Privacy of Your Health Information.

  1. Your Personal Information

“Personal information” refers to specific facts that can be used to identify or contact you as an individual. Personal Information includes, for example, your name, home address and phone number, birth date, your sex, email address or other information that is unique to you that may not directly relate to your health.

“Personal Information” does not include information that identifies a computer used to access this site because that information doesn’t identify the person using the computer.

For example, we collect Personal Information you submit when you submit a refill request, register as a new patient, make a payment, or ask us a question.

Text Message Usage

Central Iowa Family Institute maintains strict privacy policies to protect the personal information of our users obtained for text message communications. This information is never sold, rented, released, or traded to others without prior consent or legal obligation. Any sharing of information with third parties is solely for the purpose of fulfilling the organization’s obligations to the user. We guarantee that it will never be shared with third parties for marketing purposes.

Text Message Opt-Out: If you are receiving text messages from us and wish to stop receiving them, simply respond with either “STOP” or “UNSUBSCRIBE” to the number from which you received the message. Once we receive your message, you will no longer receive further text messages from us.

  1. How CIFI Protects Your Personal Information

We maintain administrative, technical, and physical safeguards designed to protect the Information that you provide on our Online Services. These safeguards vary based on the sensitivity of the Information that is being collected, used, and stored.
